Of all the developing fields of surgery, few have had the impact of vascular surgery in affecting so large a section of the population by offering the potential relief of disability and the prolongation of life. As would be anticipated, the interest of surgeons in this evolving specialty has largely been focused upon the selection of the most appropriate operations and the development of modifi cations or new procedures that overcome the limitations of previous ones. The literature contains numerous conflicting claims about the relative merits of various procedures, and those aPlJoaching this field for the first time may understandably be confused. Yet, if one were to approach any single vascular surgeon at any given time, he would upon request be given a firm and precise listing of preferred procedures. The procedures described in this two-volume publication are those that we currently favor. Their selection is the result of continuing analyses of our own experience during the past 30 years. All patients have been available for prolonged follow-up evaluation. Late failures often helped us to identify techniques that lessened the durability of an initially satisfactory operation, and they became a stimulus for the development of modifications or trials with alternative operations. Particular attention is directed to the manner in which knowledge of the natural history of disease influences the choice of operations.

Knowledge and understanding of vascular disease, particularly atherosclerosis, continue to expand across disciplines, as do diagnostic and therapeutic strategies for treatment of vascular disorders. This manual offers a consistent "how to" approach that places specific emphasis on management. Each chapter provides the reader with a highly practical approach so that he or she comes away with a reasonable amount of familiarity as to be able to manage the patient independently. Since the first edition, a certification in vascular medicine has become available. Some of the key features include: • Practical information such as drug doses, protocols for managing disorders, and algorithms; • Guidance on diagnostic assessment and treatment strategies for the majority of vascular disorders encountered in clinical practice; • Assimilation of information from areas outside cardiology, i.e., vascular surgeons and interventional radiologists, into a reader-friendly format geared toward the average practicing internist or cardiologist.

Complications of Percutaneous Coronary Intervention: The Survival Handbook aims to serve as a reference manual for all cardiologists, particularly those in training, who face life-threatening complications of PCI on a day-to-day basis. Drawing on the background and experience of cardiologists from both sides of the Atlantic, the book aims to be a valuable, practical, and most of all readable resource for the interventional cardiologist. The principles and practices described in the book will be of interest to all practitioners of interventional catheter procedures, including interventional radiologists, vascular surgeons, and cardiologists.Few medical procedures have evolved as dramatically or as quickly as percutaneous coronary intervention (PCI). From its initial conception, PCI has evolved to become a very complex procedure. Each year in the United States well over 1,000,000 percutaneous coronary intervention procedures are performed. However as the volume and complexity of the procedures being performed has increased, so has the potential for procedural error leading to life-threatening complications. Knowledge of what complications can occur during PCI and how to deal with them is vital for the practicing cardiologist, and can mean the difference between life and death.

The management of soft-tissue neoplasms has always had an aura of mystique due to the relative rarity of these lesions in comparison to the more common cancers occurring in man. There are many variations in technique of the operations employed for these tumors as well. In this volume we have illus trated our own personal preferences in approach to soft tissue sarcomas with the hope it will serve as a useful guide to the surgeon who is faced with one or more of these problems. None of the operations depicted in this manual are original with us but they are procedures and approaches we have found effective for the clinical management of this family of tumors. In presenting this volume we wish to acknowledge our gratitude to our surgical teachers, our present and past associates in surgical oncology, our colleagues in the other oncologic disciplines, and our patients whose treatment is illustrated herein. We are indebted to Dr. Saul Kay, Professor of Pathology, and Dr. James Walsh, Associate Professor of Radiology, for providing the illustrations of histopathology and radiologic imaging in the early chapters. We particularly appreciate the superb artwork of our enthusiastic teammates, Trudy Nicholson and Jane Hurd, as their contribution is the crux of our endeavor. Walter Lawrence Jr., M.D. James P. ment of those endocrine diseases encountered by the general surgeon. Opera tions on the pituitary gland are, therefore, specifically excluded. It should be most useful to general surgical residents and practicing surgeons, but we hope it will also prove useful to the wide range of physicians who deal with surgically correctable endocrine disorders. The book has four sections - Surgery of the Parathyroids, Thyroid, Pan creas, and Adrenals. Each section deals in a systematic manner with practical aspects of surgical anatomy, physiology, pathology, diagnosis, and treatment. Specific attention has been given to a detailed description of operative strategy, operative technique, and postoperative management. Individual operative steps are illustrated in full color, with specific figure legends on facing pages. The artwork represents the distillation of twelve months intensive collabora tion between the authors and a staff of medical illustrators to 'ensure anatomic fidelity and clarity. We believe that the use of full color has opened up a new dimension in surgical illustration by providing greater realism and allowing accent of important features. The techniques described herein represent personal preferences. The authors recognize that there may be several satisfactory alternate approaches in every case. Where there is controversy, we have outlined our approach to the problem somewhat dogmatically in the belief that in certain circumstances a single point of view is better than several without selection.

difficult, to represent it as the surgeon sees it, and to understand the anat omy, which is not always visible. In part, this was accomplished by paint ing on both surfaces of transparent paper to create the illusion of looking through superficial layers to the deeper structures. A combination of color media, including colored pencil, graphite, carbon, pastels, and transparent and opaque watercolors, enabled me to convey the field with minimal loss of reality or dimension. Of equal importance was the availability, for every illustration, of direct surgical observation, surgical photographs, fresh specimens, and the surgical instruments. The sequence of creation was first the discus sion of desired illustrations, then a rough sketch, consultation with the surgeons, finished pencil drawing, another consultation, and finally the color rendering. These color renderings were then checked against actual surgery for accuracy in representation of tissues, instrumentation, tissue responses to manipulation, and consistency of representation. From these processes evolved a technique that facilitated the flow of information, in logical sequence, from one step to the next and from one procedure to another, always focusing the attention of the audience toward what is pertinent and away from the extraneous.

Winner of ‘BMA Book of the Year’ in2004 Judges’ summary of the book: “This is a first-class text on the management ofall aspects of the diabetic foot. It is a practical guide withinformation and advice, which is easy to access. It takes thereader through the natural history of the diabetic foot from thenormal foot and relatively minor pathology to the end stage ofinfection, gangrene and ultimately amputation. The various stageswith the book’s structure have case studies to highlight theclinical management problems encountered by diabetes footspecialists….Much of the text is broken up with bulletpoints, chapters and sub-chapters which allow the reader to pickout information from quick scans of the book. There is liberal useof illustrations throughout the book, which is particularlyimportant as diabetic footcare is a specialty which is particularlyreliant on observation. The book also details the ‘how to doit’ aspects rather than vague reference to what is required. It also uses the published evidence for its recommendations but, where this is lacking, the authors draw on their extensiveexperience of managing diabetic feet to guide thereader.” This book is a practical clinical manual for the diagnosis, treatment - both medical and surgical - and the long-term care offoot problems in people with diabetes. The authors areworld-renowned experts in this subject and they present theinternationally recognized best practice. The clinical chapters are presented in a structured, color-codedformat to allow easy reference. The text also includes highlightedbest clinical practice tips. Each chapter is accompanied byhigh-quality, full-color photographs that provide excellentexamples of the earliest presenting signs as well as illustratingthe step-by-step guidance on treatment. This book is an essentialmanual for every healthcare professional involved in the care ofpatients with diabetes.

As research in liver disease has greatly expanded over the past few decades the etiology, pathology, and natural history of many hepatic disorders have been clearly defined. Many new and accurate diagnostic tools are now available. In spite of all this progress the treatment of many liver diseases remains ameliorative. Nevertheless a more aggressive and successful surgical approach has taken place in regard to many diseases of the liver in part due to the pioneering work of Dr. William P. Longmire, Jr. and his colleagues. Dr. Longmire was among the early advocates of liver resection of neoplasms, successfully remov ing a large benign tumor of the liver in 1946. In the following decades his contributions covered the surgical treatment of trauma, tumors, cysts, and other congenital abnormalities. He has performed hundreds of operations on patients with these disorders. In 1948 he reported a procedure now named for him, in which intractable common duct obstruction was relieved by partial left hepatectomy and cholan giojejunostomy. He was one of the first to advocate side-to-side portacaval shunt for portal hypertension. Other essential contributions by this master surgeon were the periodic reviews that covered the advances in his field. Dr. Longmire, with his scholarly approach and broad background of knowl edge in all aspects of hepatic surgery, is exceptionally qualified to write this book. He and his colleague, Dr. Ronald K. Tompkins, are to be congratulated on this complete treatise.

Emergency vascular and endovascular practice comprises almost half the workload undertaken by the majority of vascular surgeons. Most vascular emergencies are characterised by the sudden occlusion, rupture or injury of a blood vessel placing organ, limb and life at risk. An attitude of urgency and of good judgement, both in diagnostic assessment and the application of evidence-based vascular and endovascular techniques, is crucial to the achievement of optimal outcomes. This book is structured with the emphasis on clinical presentation, the objective being to provide accessible and highly practical advice to the vascular surgeon faced with a particular emergency. Distinguished vascular surgeons, vascular radiologists and other specialists around the world have enriched this book with authoritative contributions reflecting their experience and expertise. For ease of reference the text is divided into nine sections, leading with introductory chapters on general considerations such as pathophysiology, preoperative diagnosis, risk assessment and medico-legal aspects of vascular emergencies. That is followed by a series of clinical sections covering acute cerebrovascular syndromes, acute limb ischaemic states, the acutely swollen limb, thoraco-abdominal catastrophes, acute complications of endovascular aortic repair, regional vascular trauma and iatrogenic injuries. It concludes with a section on miscellaneous, but important, acute vascular challenges. Throughout the book, wherever appropriate, modern endovascular techniques are given full coverage. International in its approach, this book will fast become established as the text devoted to the surgical management of vascular and endovascular emergencies. It is essential reading for vascular surgeons and radiologists and a useful reference source for general and trauma surgeons, angiologists and emergency physicians.
